BMJ followed 550 participants over 30 years, controlling for 26 potential confounders. Higher alcohol consumption was associated with 3× the odds of hippocampal-volume markers (right hemisphere), differences in lexical-fluency measures, and reduced white matter microstructure. The study found no supportive effect at any consumption level — adding nuance to the "J-curve" hypothesis of moderate alcohol benefit on brain health.
